All currant players as well as new players to the Club are required to fill out a new Registration and Emergency Medical Release Form for each soccer year. (Sep 01 to Aug 31 the following year) Forms can be obtained from the coaches or can be copied from the links below. Training release form is for players attending the Y clinic but not registering with the Club for Summer/Fall league play.
Player Registration & Emergency Medical Release forms can now be typed out in Acrobat Reader and then printed out. Please be sure to sign the finished forms. The MYSA match line up can also be typed in and printed.
